In purely biochemical terms, the drug performed its designated task. It successfully lowered lipoprotein(a) blood levels. But biology stubbornly refused to cooperate with the spreadsheet models. That reduction did not translate into fewer cardiovascular events, leaving the Swiss pharma group staring down the barrel of a primary endpoint miss.
Chief Medical Officer Shreeram Aradhye admitted the obvious disappointment, noting the results fell short of expectations while spinning the data as a learning exercise. Yet this gap between a moving biomarker and actual patient outcomes opens up deep structural vulnerabilities across the entire cardiovascular pipeline. Rivals like Amgen and Eli Lilly are pushing their own alternatives targeting lipoprotein(a) through entirely different biological pathways. They now face an existential question. If lowering this specific lipid particle does not protect the heart, the race to capture a market affecting one in five people worldwide suddenly looks far more hazardous.
As Jefferies analysts pointed out, the rising bar set by existing therapies and GLP-1 weight-loss drugs means new cardiovascular treatments face an unforgiving commercial landscape. Compounding this pressure, Novartis is managing the fallout from pausing eight trials of an experimental cell therapy following three patient deaths, all while trying to extract value from a twelve billion dollar bet on Avidity Biosciences. The era of easy victories driven by proxy markers is ending. Companies can no longer rely on tweaking laboratory values to justify inflated valuations when the ultimate test of human survival refuses to yield.
